The Title Agreement Form should only be submitted if you have chosen to use a sponsor's name as your competition title. Both parties must agree.

We, the aforementioned Company, have agreed to be a financial sponsor for the contestant listed above. In exchange for our sponsorship, this contestant agrees to bring recognition to our company as her title sponsor. Our company name will appear on her competition sash, in the program book, and will be announced from stage each time she is presented. She has our permission to use our company name as her official title for competition purposes and all that entails.
