In consideration of participating in the Miss USA and Miss Teen USA State Pageant listed above and its related events and activities, I agree that:

1. There are inherent risks of injury.

2. I knowingly assume those risks and agree to indemnify and hold harmless, the Miss USA/Teen USA System and Vanbros and Associates, Inc. for all injuries sustained.

3. I am soley responsible for all the items I bring to the pageant. Any item lost or stolen is not the responsibility of Vanbros and Associates, Inc.

4. I am soley responsible for any damage I may cause to the pageant venue.

5. I am soley responsible for safety precautions regarding my personal health and the risk of contracting or spreading any virus including COVID-19 or related variant of COVID-19.

I have read this release of liability and assumption of risk agreement, fully understand its terms and sign it voluntarily. I understand that if I am under the age of 18 I must have a parent or guardian sign this form on my behalf.
